2009 Summer Heat Relief Program

Fans and Air Conditioners Available for Residents with Medical Conditions


Citizens who suffer from certain medical conditions may be eligible to receive a free fan or air conditioner through the Louisville Metro Community Action Partnership (LMCAP) and the WLKY TV-32 Fan Club. The Summer Heat Relief Program is available through August 28, 2009.

The program provides one air conditioner per household for family members with certain medical conditions – those deemed life-threatening or which may worsen without air conditioning. To qualify, applicants must meet the 125 percent federal poverty income guideline.

Applying is a two-step process. First, applicants must have their doctor fill out a medical form, and then contact CAP to schedule an appointment to provide identification documents.

Applicants may call or visit a LMCAP location to receive a medical form to be completed by their doctor:

· CAP Administrative Office – 810 Barret Avenue, Room 127, 574-1157

· CAP East Office – 4810 Exeter Avenue, 574-1270

· All Neighborhood Place locations

Applicants may call 574-1270 for an appointment once their medical form is completed. All appointments are assigned from 9 to 11 a.m. Monday through Friday.

A Medical Review Committee meets each week to determine which applicants will be awarded an air conditioner. All applicants who do not meet the criteria to receive an air conditioner will receive a fan.

Applicants must bring the following to their appointment:

· Picture ID

· Proof of Social Security numbers for everyone in the household

· Proof of income for the previous month

· Proof of address

· Completed original medical form with doctor’s signature

Applicants who have received an air conditioner or fan within the past five years are not eligible for the program. For more information, call 574-1270 or visit www.louisvilleky.gov/cap.
