Information and Referral (I&R) staff performs a variety of services to clients at Neighborhood Place.
- Assesses clients for financial eligibility
- Makes referrals to other community-based organizations when appropriate
- Educates community on additional resources inside/outside the walls of Neighborhood Place
- Identifies gaps in services
Case Management Services
Neighborhood Place Case Management Services are available for families, individuals, and youth to develop strategies to improve their functioning while maximizing their strengths and opportunities. In addition, our case management services assist in achieving economic independence and stable housing by providing assistance to suitable employment, increasing household assets, budgeting, and linkages to financial institutions.
Human Services Case Management Grant – provides case management services to 30 homeless and disabled individuals who are in permanent supportive housing funded by other federal sources.
Self-Sufficiency Program – provides case management services to the participants in the HOME TBRA program to address the issues that will lead them to self-sufficiency.
Through Any Door
Through Any Door (TAD) is a partnership between Kentucky’s Cabinet for Health and Family Services, the Food Stamp Program, local faith and community based non-profit organizations, and Louisville Metro Government. The purpose of this initiative is to:
1) Implement technology that directly impacts the application, certification, case management and recertification for Food Stamps and
2) Use a “through any door” strategy that makes these processes easier and more accessible for applicants and recipients. TAD will screen and apply for other benefits simultaneous with Food Stamps, and other community resources.
