Chronic Disease Prevention

The Chronic Disease Prevention Team is part of the Community Health Education and Promotion Division of the Louisville Metro Health Department in Louisville, Kentucky. The team consists of two nurses and a dietitian who are Certified Diabetes Educators, another dietitian certified in adult weight management, a health educator and a secretary.

They offer preventive services related to cardiovascular disease and diabetes, including a variety of Health and Wellness Classes.

Education is important in preventing diabetes and cardiovascular diseases. Learning more about diet, exercise, tobacco use and other lifestyle choices, and modifying behavior accordingly, can also   help to control and treat these diseases in order to reduce the risk of complications.
