Performance measurement is a reporting tool that will provide various departments within Metro Louisville Government and its many partners throughout the community the ability to “tell the larger story” of why their programs have meaning and how these programs improve lives in our community. This tool will provide us with the ability to meet Federal requirements of collecting compulsory data, tracking progress, and also reporting on program results and their impact on the Metro Louisville community.
Effective January 2009, the Louisville Metro Department of Housing & Family Services will require the use of the forms listed below for all HUD-funded (HOME, CDBG, ESG and HOPWA) programs in a concerted effort to obtain required reporting measures mandated by HUD.
