For projects receiving other Louisville Metro funding,
and have less than 10 units 
 

Monthly Reporting Documents

The Monthly Reporting forms must be completed monthly, for 6 months, during the project’s initial rent-up stage.  Monthly reports must be submitted to the Louisville Metro Human Relations Commission office by the 15^th of each month, as applicable.
