Report A Broken Sidewalk

Do you have a sidewalk in front of your home or in your neighborhood that you feel is in need of repair? If so please fill out the form below so that we can submit the request to Metro Public Works.*  

Once public works receives this information, an inspector will be sent out within 15 to 30 days. The inspector will examine the sidewalk and assign it a rating between 1 and 5, with 5 being the worst.

District 11 funds will be used to repair all sidewalks that have been given a rating of 4 or 5 as quickly as possible. Any sidewalk that is rated a 3 or below will be fixed when funds become available.
