The Award
Winners will be honored at the 2008 Mayor’s Annual Youth Service Worker Awards Ceremony which will be at Memorial Auditorium from 7 - 9 p.m. on Tuesday, April 1, 2008.
The winners will receive:
A glass plaque representing the award;
A framed 5x7 picture with the Mayor;
A framed award signed by the Mayor;
A feature on MetroTV spotlighting the winner and organization.
Submissions
Use one form per nomination. Photocopies are permissible for additional nominations. Please include all pertinent information about your nominee. Send in the original and three copies. Deadline
Nominations must be received by the Office of Youth Development by close of business March 5, 2008.
Winners will be notified in March and announced at the 2008 Mayor’s Youth Service Worker Awards Reception in April.
Eligibility
Nominations should reflect how the nominee adheres to the following:
• Appreciation and Respect for Youth
• Listening Skills
• Flexibility with a Positive Attitude
• Clear Expectations
• Overall Commitment to Youth
In addition, the supervisor of the nominee must certify that the nominee works for the organization and is in good standing. A background check must be completed and on file
Judging and Scoring Guidelines
A panel of judges (comprised of individuals with significant youth work experience as well as past award winners) will review each nomination. Each narrative description has the potential of receiving 20 points for a total score of 100. The quality and clarity of the narrative descriptions are very important to the judging panel. Please consider the nominee’s unique contributions and special relationships with youth as well as how the nominee “makes a difference” with the youth he or she serves. Awards are based on the highest average score and all decisions are final.
