Nominee’s Narrative:
Describe accomplishments fully and list any characteristics that might distinguish your nominee from others. Narrative should focus on accomplishments over thelast year but may include historical information. Text from this narrative will be used for press releases and the awards ceremony.
Supporting Material:
Nominator may attach a maximum of two additional pages of supporting documentation such as photos, newspaper clippings, letters of support, etc.
Nomination Includes:
• Data sheet signed by nominee, nominator and nominee’s supervisor
• Nominee Narrative
• Additional Supporting Information (if any)
Please provide original and three copies of the nomination
Nominator understands that:
• Nominations become the property of Department of Neighborhoods,
Office of Youth Development and will not be returned
• If nomination is handwritten, the information must be legible
• Application must be submitted by March 5, 2008
• Permission from the nominee must be obtained
• Award winners may be asked for additional information
