The Tobacco Prevention and Cessation committee will convene on topics related to reducing the prevalence of tobacco use among adults and youth and decrease youth initiation rates. Partners will promote efforts towards making Louisville smoke-free.

The Chronic Disease Prevention and Management committee will convene on topics related to improving chronic disease prevention and management by increasing the percentage of adults receiving age appropriate screenings (mammography, pap smear, blood cholesterol, etc).

Led by the Louisville Metro Department of Public Health and Wellness (LMPHW), Healthy Hometown unites diverse community partners from business, schools, government, academia, neighborhood groups and non-profit organizations to coordinate activities that will increase physical activity and healthy eating to improve the health of Louisville Metro residents.
