Dance Hall Application (PDF)
- Applicant must provide criminal history transcripts from the Records Section of Louisville Metro Police Department, 768 Barret Ave., Hrs: Monday thru Friday, 8:00 am to 5:00 pm. There is a $3.00 charge for this information. If applicant has resided outside of Jefferson County within the past three (3) years, criminal history information must be provided for all areas in which the applicant has resided in that time period.
- If not already registered, the applicant must register with the Louisville Metro Revenue Commission, 101 S. 8th St., Hours 8:30 am to 4:30 pm. Applicant must be in good standing with the Revenue Commission.
- If applicant is a corporation, a corporate addendum must be attached and corporation must be in good standing with the Kentucky Secretary of State Office.

- If applicant is not the owner of the property, a lease must be provided going through the licensing year.
- The applicant may submit the above listed items, along with a completed application and $100.00 licensing fee to the Department of Inspections, Permits and Licenses, 444 S. 5th St., Mon. thru Fri., 8:00 am to 4:30 pm. Or you can mail it to this address.

License will be granted upon verification of compliance with all requirements, including adequate zoning (premises must have at least C-2 zoning or non-conforming usage) and approval from the Division of Fire.
